Milton Primary Care Awarded Top Performer in Re-admission Prevention

Milton Primary Care was awarded the number one Top Overall Performer in the Re-admission Prevention Award Program by Tufts Health Plan for their Tufts Medicare Preferred work. They achieved a low 3.5% re-admission rate for 2013. Along with being the number one top performer, they were also recognized for Greatest Improvement.

Each year, Tufts Health Plan awards the top three physician groups in overall performance, and year-over-year performance improvement, with public recognition and cash awards. This year’s awards were announced at the September 2014 Medical Director Meeting.

Using the CMS Star Rating Plan All-Cause Re-admission measure, consistent with HEDIS, the measure allows for comparisons of providers by calculating an observed-to-expected ratio, which serves as a severity adjustment. From here, the final rate is calculated by multiplying the provider’s observed-to-expected ratio by the national average observed rate. To qualify for this award, a continuous enrollment 12 months prior to index discharge date and 30 days following index discharge date and admissions to a behavioral health facility must be shown.
