Linking Adults with Behavioral Health Support
Awarded by Harvard Pilgrim Health Care, May 2016 - ongoing
Behavioral Health has become an area of focus within the NEQCA network. Building upon the already successful Pediatric Behavioral Health program, NEQCA will utilize the $75,000 it has been awarded to pilot an innovative approach to support behavioral health for adult patients, build capacity and address behavioral health needs on Cape Cod, while partnering with Gosnold, a nationally accredited addition and mental health treatment provider.
Meeting Cultural and Linguistic Diversity Needs

Health disparities often correlate with socioeconomic and ethnic categories. NEQCA is partnering with Merrimack Valley Independent Practice Association (MVIPA) practices, Lowell General Hospital, Merrimack Valley Area Health Education Center, NEQCA Complex Care Managers and Care Managers from Lowell General Hospital to focus on cultural and linguistic competence. The collaboration will develop a cultural competency training program for practice teams and care managers, introduce mobile technology for interpreter services and identify effective patient-facing health education materials to support patient adherence to a plan of care.
Training Care Managers to Provide a Consistent Model of Care
Awarded by Commonwealth Corporation Healthcare Transformation Fund, December 2014 - ongoing
NEQCA in partnership with Tufts Health Care Institute has been awarded a $250,000 grant. The funding will help to develop and deliver a comprehensive training program for our care managers. The training will strengthen the knowledge and skills of our care managers and improve the outcomes of their interventions with patients and practices. The new curriculum includes motivational interviewing, documentation standards and requirements, behavioral health training, workflows, “warm-handoffs” during introductions between providers and patients, and transitions of care. The implementation of the training will strengthen the care managers’ capacity to provide patient-centered care and integrate behavioral health care with medical services.
Pediatric Behavioral Health Social Workers
Awarded by Harvard Pilgrim Health Care, May 2014
Behavioral health is an area of increasing concern across the network. This grant supported coordinated efforts of a Behavioral Health Coordinator at three pediatric practices in the Highland LCO. This work has helped NEQCA understand the resources and protocols necessary to expand this effort more widely across the network. With the success of this pilot, NEQCA has expanded the program and brought on 3 addition Behavioral Health Social Workers, covering practices on a regional basis.
Managing Heart Failure in the Elderly
Awarded by the Verizon Foundation, February 2014 - ongoing
Tufts Medical Center, in collaboration with NEQCA have been selected by the Verizon Foundation to receive $150,000 in funding to support the management of elderly heart failure patients by utilizing state-of-the-art tablet technology along with telephonic disease management.
Using tablet computers, teleconferencing and other communications technology to ensure that patients are always connected to their care team, the Verizon grant aims to support the improvement of quality and cost-effectiveness of heart failure disease management. As we know, communication between patients and their care team is essential for disease management and implementing this technology will enable caregivers to more effectively communicate with patients, promote self-management, and ultimately decrease the risk of heart failure among seniors.
